As an employee of an agency that provides waiver services, you are required to meet the qualifications and requirements specified in the Core Assurances for Providers of Developmental Disabilities Home and Community-Based Waiver Services Program. You may obtain this document in the form of a .pdf file in order to read the information online or if you prefer, you may print and read the document. When you have finished reading the information, download and print the .pdf file for the questions, answer the questions, check your answers, and then submit your answers to the office for filing in your personnel file.
